Predictive Analytics Care Model
We follow a care model that is customized to determine a patient’s risk levels upon release to a post-acute care facility. Using predictive analytics tools, providers generate real-time data for care plan decision-making. If needed, mobile unit visits for high risk patients discharged during low staffing periods are available.
