Over the past month, nurse practitioner Julie Miller, Legacy Care’s Chief Compliance Officer, hosted several continuing education events on transitional care management for several Florida Healthcare Association chapters around the state.
“1 out 5 patients discharged from the hospital will be readmitted,” said Julie. “75% of these readmissions could have been prevented.” Julie shared other insights, including the top reasons for readmission and recommendations on how to manage.
