Yolanda Hoag, ACNPC-AG, was appointed to serve as a Legacy Care Transitional Care Advanced Practice Provider.
In her new role, Yolanda will steward the next generation of care delivery by bridging the transitional care gap across the entire care continuum, by deploying LGS Legacy Care’s analytically-based care model leveraging artificial intelligence from multiple verticals. The LGS Legacy Care Model – focused on reducing readmissions and optimizing length of stay – will be the focal point and complementary measure to evidence-based and customized clinical protocols.
Yolanda will partner with LGS Legacy Care’s acute, post-acute, long-term and home healthcare high performing partners, ensuring patients are provided the level and quality of care above and beyond regulatory and consumer expectations.
For more than a decade, Yolanda has been a clinical leader and pioneer in delivering care across all aspects of the patient’s journey of health. Her clinical expertise consists of Emergency, Prenatal, Family Planning, Ambulatory, Post-Acute and educational environments. Prior to healthcare, Yolanda served as the Operations Sergeant for a Brigade Commander consisting of six battalions with over 6,100 soldiers. She completed several oversea tours, where she worked as a clinical instructor of combat lifesaving and navigation.
Yolanda received her Bachelor’s of Science in Nursing from McKendree University in Illinois and a Master of Science in Acute Care Adult Gerontology from Walden University in Minnesota. She completed her Acute Care Nurse Practitioner fellowship through the California Emergency Physician residency program.
